Hazlehurst Elementary Pre-K and K-5 registration set

Hazlehurst Elementary School will hold Pre-Kindergarten and Kindergarten registration May 15, 2009, from 9 a.m. to 2 p.m.

Please bring the following information to make your child’s packet complete.

1. Birth Certificate-original long form

2. Social Security number/card

3. Form 121-Immunization Shot Record

4. Two (2) proof of residency documents, which could include utility bills, vehicle registration, copy of lease or mortgage.

All students that are registering for Pre-K need to accompany their parents at registration.
