Mt. Sinai Center accepting applications

The Mt. Sinai Head Start Center, located behind the  Hazlehurst Middle School is now accepting applications for three (3) and four (4) year olds for the 2014-2015 school term. The required documents needed for enrolling are: long form certified birth certificate, Form 121 shot record, proof of income 1040 tax return form for 2013, all family members social security numbers and Medicaid number of the applicant if applicable.
